Dental owner · AI citation gap

OPEN. UNSEEN.

You take same-day emergency appointments, and ChatGPT sends new patients three blocks away.

The buyer typed "dentist accepting new patients in [your city]" or "emergency dentist near me with sedation" at 8pm. AI named three local practices. You opened a new chair last quarter. You are not cited.

What this page covers

The six layers of this read.

  1. Why AI started recommending other dental businesses instead of yours
  2. The pattern: how AI builds the dental shortlist
  3. What you have already tried that did not move the citation
  4. Diagnostic questions for the AI search gap
  5. Stan's take on the citation fix
  6. Common questions before the BUILD starts

The buyer asks AI. AI names three. You are not on the list.

The migration from Google search to AI search hit local-trade categories starting in late 2024. Buyers now type the question conversationally; AI returns a short list of named businesses; the buyer picks from that list. Four mechanics decide which businesses get on the list.

Pattern

New-patient queries are the highest-intent AI dental searches.

Buyers asking AI for a dentist are typically new to the area, transitioning insurance, or in pain. The AI citation arrives at the moment the buyer is most willing to book. Practices not cited at that moment lose the buyer to one of the three the AI named.

Pattern

Insurance-acceptance signals are part of citation.

AI cites practices whose insurance-accepted list is structurally visible. Practices with insurance information buried in a footer or only accessible via phone call get filtered for insurance-specific queries that buyers ask routinely.

Pattern

Specialty markers (sedation, pediatric, cosmetic) are citation differentiators.

"Best dentist for nervous patients," "pediatric dentist in [city]," "cosmetic dentist for veneers" are specialty queries with low competition. Practices with the specialty marked clearly in schema and content win these citations even against larger general practices.

Pattern

Same-day availability signals raise citation rate on emergency queries.

Emergency dental buyers want an appointment now. Practices that signal same-day availability in schema and on the homepage outrank practices with appointment-by-phone-only language for emergency queries.

AI does not judge dental businesses. AI surfaces them. The businesses with the right structural signals get surfaced; the businesses without them get filtered before any human sees a result.Pattern observation · Stan Consulting

AI does the comparison. Three brands cited. The other businesses vanish.

Stage 1: buyer asks AI conversationally. Stage 2: AI cites three named dental businesses with supporting points. Stage 3: buyer contacts one of the three. Businesses not cited at Stage 2 never enter the decision. Their reviews never get read. The funnel happened upstream.

60-120days

Most local businesses see their first AI citation appearances within 60 to 120 days of the BUILD shipping.

The structural signals get re-indexed by ChatGPT, Perplexity, and Google AI Overviews over rolling cycles.

Citation share compounds through the next two quarters.

Four phases. Thirty days.

01

Discovery

30-min call. Site audit. Citation baseline.

02

Buyer prompts

20-40 real queries captured. Engine tested.

03

Install

Schema, llms.txt, entity, content pages.

04

Measure

Citation re-measurement. Written report.

ENGINEERED. NOT EARNED.

Three rules. One install.

01

Buyer language wins citation. Category language loses it.

02

Schema beats content volume at the retrieval step.

03

Editorial citation compounds; reviews alone no longer originate.

When operators ask why their best work is not showing up in the AI answer, the answer is almost always that the AI cannot read what is not structured. The work is real. The signals are not.Stan Tscherenkow · Principal · Stan Consulting

Four moves that did not put you on the AI list.

Dental owners try the standard fixes first. Each one improves something else and leaves the AI citation gap untouched.

What was tried

What you tried

  • Boosting Google Reviews count with patient follow-up campaigns
  • Investing in higher-end before/after photography
  • Running Facebook ads in your zip codes
  • Refreshing the website with new doctor bios
  • Paying for ZocDoc premium listings

What closes the gap

What gets you on the AI list

  • Dentist schema (Dentist + LocalBusiness) with services, insurance accepted, specialties
  • Buyer-prompt content for new-patient onboarding, emergency, sedation, specific procedures
  • Same-day availability signaled in schema and homepage hero
  • Insurance-accepted list as structured content the AI can parse
  • Editorial citation on local health publications and parenting/family magazines

The diagnostic. Six questions.

If three or more answers point the wrong direction, the pattern is structural, not effort-based.

  1. Ask ChatGPT "dentist near me accepting new patients with [your insurance]." Are you named?
  2. Is your insurance-accepted list structured content or buried in a footer?
  3. Do you have a buyer-prompt page for each major specialty you practice?
  4. Is your same-day or emergency availability signaled clearly?
  5. Has a local health publication or family magazine cited your practice in the last 18 months?
  6. Are your accepted insurance plans visible in schema markup?

Stan's take

AI citation is engineered. The dental businesses doing it are absorbing the new leads.

Dental practices have been told for a decade that Google Reviews are the trust lever. They were. They still are at the stage-two filter. The stage-one filter is now AI citation, and review count alone does not move it.

The practices winning AI citation in dental in 2025 are the practices that did three things: cleaned their insurance-accepted markup, built specialty-specific buyer-prompt pages, and signaled same-day availability in the structural layer. Each is a 1-2 day fix individually; together they take 30 days to install correctly.

What changes after the install: when a buyer asks AI for "dentist accepting new patients with [insurance] near [city]," your practice has a measurable chance of being one of the three names. Before the install, practices with cleaner schema win regardless of clinical reputation. That is the system; it does not care about reputation it cannot read.

New-patient acquisition is the most expensive line in a dental practice. AI citation lowers that cost structurally because cited practices are getting pre-qualified buyers who already chose to call. The math justifies the install inside one new-patient acquisition.

What operators ask before the first call.

Does this work for general dentistry or just specialty practices?

Both. General practices benefit from new-patient and emergency citation; specialty practices benefit from specialty-specific citation (orthodontics, oral surgery, pediatric, cosmetic, sedation).

What if my practice is solo vs multi-doctor?

Both work. Solo practices often win specialty citations faster because the practice-doctor entity is unambiguous. Multi-doctor practices benefit from Person schema for each provider.

Will this conflict with my ZocDoc or 1-800-DENTIST presence?

No. Third-party directory presences become supporting entity-clarity signals. The BUILD does not require you to remove them.

How does insurance-accepted markup work for HMO + PPO + Medicaid practices?

Each accepted plan is structured as a separate entity in schema. The BUILD installs the full accepted-plans list with proper markup.
